Verification

If you are an Undergraduate student and have been selected for the verification process, please be advised that we may request any of the following documents listed below to be submitted:

  • Dependent Verification Worksheet
  • Independent Verification Worksheet
  • Identity Statement of Educational Purpose
  • Tax Return Transcript/Tax Return Forms

Graduate/Professional students are not typically selected for the Verification Process. However, if you are selected for verification, you will only be required to complete an Identity Statement of Educational Purpose for the Academic Year. The statement must be completed and signed by a Notary.

You can find and complete the Verification Worksheets online at Dynamic Forms. If you have not done so before, you will need to create an account before you are able to complete the form.

The Identity Statement of Educational Purpose must either be completed in the Financial Aid Office or you must complete the form with a Notary. Please be advised that this is also the case with any documents that pertain to proof of citizenship.

It must be mailed to St. Thomas University along with an original copy of an unexpired ID to:

If you did not or were ineligible to use the IRS Date Retrieval Tool, you can request the Tax Return Transcript by clicking here. You can also use the IRS’ Transcript Automated Service by calling 800-908-9946. It takes approximately 5-10 business days for the transcript to be mailed to the address on file with the IRS.

Please allow the Financial Aid Office approximately 72 hours to process your submission upon receipt. The estimated processing time may change depending on the amount of documents received during the time of submission. Please submit the request documents prior to the start of the semester to guarantee the maximum Financial Aid availability.
