Attendance Requirement: Certification will be granted to those participants that meet the minimum attendance requirement of 80% of class time. Certificates will be sent to participants via email to the email address provided on this form.

Once you submit this form with payment, you affirm that all information provided on this form is true and correct.  Failure to complete this form truthfully and accurately may result in denial of admission to the Human Trafficking Academy.

The Academy will be held remotely as a livestream event and must be viewed by participants in real time. Course materials will be provided electronically. A link for the live sessions will be provided to participants to view each course synchronously via Zoom (no recordings will be available once the live session has ended). The ability to ask questions to instructors will be available through a Q&A format.

No individual or entity may record, upload, share, broadcast, copy, store, distribute, etc., any portion of the Academy’s presentations, curriculum, PowerPoints, etc. (“Academy Materials”) without the Academy’s prior written consent.  Unauthorized use of any Academy Materials without the prior written consent of the Academy is strictly prohibited. 

The opinions, perspectives, and information presented represent the Instructors’ views, and do not necessarily reflect the official position of the Academy.

The Academy reserves the right to deny anyone admission to the Academy.

The Academy reserves the right to change/modify the Academy as needed.
