IMMUNIZATION RECORDS

What forms do I need to submit to the university?

ALL Students: STU requires that ALL students taking classes on campus upload documentation of the following immunization forms:

  • Measles/Mumps/Rubella (MMR)
  • Hepatitis B (Hep B)

On Campus Housing Students must upload all documentation listed above in addition to the following immunization:

  • Meningitis (MCV4)

Where do I submit these forms?

STU students should upload their immunization records and physical examination to their Admissions Portal (hyperlink: stu.edu/portal).

What if I don’t have a copy of my immunization records?

If you do not have a copy of your immunization records, please contact the Florida Public Health Department at 954-467-4943.

What if I am missing vaccines?

To receive your immunizations, you may visit your private health provider, or make an appointment at a local pharmacy. Find a list of pharmacies near our campus below:

HEALTH INSURANCE

Am I automatically enrolled in the school’s health insurance?

Yes, each year, ALL Bobcats are automatically enrolled in the school’s health insurance, United Health Care Student Resource. If you already have your own health insurance, please be sure to keep an eye out for e-mail instructions on “how to waive out of coverage”. You will need to answer a few questions and submit proof of insurance to qualify to waive out of the school’s health insurance. If your waiver is approved, the University will be notified, and your student account will be updated to deduct the school’s health insurance cost. (NOTE: all e-mails are sent to your @stu.edu email and not personal e-mails).

When do I have to waive out of coverage by?

The insurance waiver deadline for 2024 is August 9th. Please see the Student Health Insurance tab (hyperlink: https://www.stu.edu/students/student-affairs/health-wellness-center/student-health-insurance/) for the most up-to-date information and deadline.

If I waive out of coverage once, will I have to waive out again?

Yes, you will need to waive out of coverage each school year.

Am I required to have health insurance?

Yes, ALL students are required to have health insurance, whether that is through United Health Care Student Resource or your own insurance. If you carry your own insurance, you still need to waive coverage and submit proof of insurance to your Admissions Portal (hyperlink: stu.edu/poral).

 

 

COUNSELING SERVICES

 What are your rights as a student seeking counseling?

  • To be treated with respect and dignity.
  • To ask and understand how the counselor works or what their experience is.
  • To expect the counselor will keep what you tell them confidential to the service unless concerns about your safety or the safety of others.
  • To stop attending counseling at any time for any reason.

What about my records?

All health information is kept on a secure medical database.

The notes taken by the counselor are confidential and may only be seen by the Health & Wellness Center staff if there are safety issues to be considered. You may ask to see these at any time or have a copy of them if you wish. Once counseling has stopped, they are stored in the electronic health database.

How can you give feedback or voice a concern about the counseling or health service?

If you would like to give feedback both positive and negative, or you have a concern about any aspect of the counseling you have several options: You may –

  • Discuss the matter with the staff member concerned (either on your own or with a support person).
  • Contact the Vice President of Student Affairs.

Do I need to make an appointment, or do you provide a drop-in service?

Appointments are preferred to avoid waiting unless the visit relates to a crisis matter.

How much will it cost me as a student to see the counselor?

There is no cost to see the counselor on campus.

Can my partner (non-student) use the service?

Clinic services are available to registered students only.

Do I need to be referred to a counselor or can I just make an appointment?

You can contact us directly/self-refer. It is preferable to make a booking in advance.
